We are certified Geriatric Care Managers and Licensed Clinical Social Workers. We provide mental health therapy along with care management which includes attending medical appointments, referrals and resources, communicating with families and providers, and patient advocacy. We offer a holistic approach as we focus on the person as a whole to incorporate not just the diagnoses, but the environment as well. We work with individuals, caregivers, and families, especially of those with aging issues, Dementia (such as Alzheimer’s), cognitive impairment and caregiver burnout.
