Patty's Place Concierge Youth Mental Health Care serves youth 10-26 years of age and their families. Our model is designed to help individuals navigate the often complex world of accessing support. Our professionals offer a thorough needs based assessment during the Initial Intake Meeting and recommend a personally tailored plan of care. This plan may include solution-focused counseling, family mediation, coaching, referral to specialized services (i.e. psychiatric evaluations and medication management) parenting support and education. When individuals begin their journey to wellness we guide them based on their specific needs. View 8 Photos Our unique model offers timely, personalized and compassionate care. We believe that seeing people in their time of need helps to reduce negative symptomology and helps to avoid future crisis situations.
