Currently accepting Intakes only. Intake evaluations are solely for individuals seeking services for diagnosis including Anxiety disorders, Depression, Obsessive-compulsive disorders, Eating disorders, Adjustment disorders, ADHD, Conduct disorders, and Trauma related disorders. If requesting copies of your documentation, they will be available upon request and completion of consent to release. This will be discussed in depth during the intake. Please allow at least 3 business days following the date of your intake for preparation of documentation. Additionally, I can assist with internal referrals for long-term therapy or you may choose to identify a mental health provider independently. During this intake appointment, you will receive a comprehensive, client-centered interview that includes questions related to your educational, medical, mental, and social history.
